Activation is different in InDesign and Photoshop/Illustrator/Express. Details on how to activate on each can be found at this link
Yes. For InDesign, silent activation and deactivation options are also available. You can use .serial files to automate activation tasks. Reference: Silent Installation and Activation
You can download installers for the different plug-ins here:

We support the last three versions of the Creative Cloud. You offer installers for some products for older versions as well.
When your subscription expires, the plug-in switches to demo mode and stops functioning fully. Renew your subscription to continue using it. See: Logging in and purchasing
